This privacy policy sets out how Precision Dental Design Laboratory uses and protects any information that you give when you use this website and when you receive a dental product from us under the care of the dentists we work with.

The laboratory is committed to complying with the General Data Protection Regulation (GDPR), GDC and other standards.
The laboratory only keeps relevant information about employees for the purposes of employment, and about patients to provide them with safe and appropriate dental care.

 

The person responsible for Data Protection is Mr Muller van der Westhuizen.

We are the sole owners of the information collected on this site. We only have access to/collect information that you voluntarily give us via email or other direct contact from you. We will not sell or rent this information to anyone.

 

We will use your information to respond to you, regarding the reason you contacted us. We will not share your information with any third party outside of our organisation, other than as necessary to fulfil your request, e.g. to ship a repair.

 

What personal information do we need to hold?

  • We need to hold your name or patient identifier, date of birth, prescription information, dentist name and dentist address

  • We will need to keep information about the device we have provided, our self-declaration of conformity and labslip relating to your care.

  • Notes of conversations or incidents that might occur for which a record needs to be kept.

  • If you visit us for a repair we will also need to keep a contact phone number.

 

Why do we hold this information?

We keep accurate personal data about patients in order to provide you with appropriate and safe devices. We also need to store personal data about you and to ensure proper administrative records.

 

Retaining Information

We are required to retain these records for ten years.

 

Security

 Information about you is stored in the Precision Dental Design Laboratory computer system and/or in a secure manual filing system. The information is only accessible to authorised personnel.
Personal information will not be removed from this laboratory without the patients authorised consent.
Your personal information is protected by the staff at the laboratory.
All access to information is held securely and can only be accessed by password.
Data is encrypted, and computer terminals are password protected when unattended.

This includes:

  • Password protection

  • Locked cabinets/rooms

  • Virus protection

  • Secure cloud-based storage

 

Access

 You have a right to access the information that we hold about you and to receive a copy.
You can make a request by contacting the laboratory or by e-mailing pddlaboratory@gmail.com

 

Rectification

You have a right to correct any information that you believe is inaccurate or incomplete.
Please contact your practice to request a change in information.

 

Erasure

You have a right to request that we delete your personal information, although you should be aware that, for legal reasons, we may be unable to erase certain information (for example, information about your dental treatment). Please contact your practice to make this request.

 

Restriction

You have the right to request us to restrict the processing of your personal information for example, sending you reminders for appointments or information about our service. Please contact the laboratory to make this request.

 

Portability

You have a right to data portability, this could include supplying your information to another dentist. Please contact the laboratory to make this request.

 

Concerns

If you have any concerns about how we use your information and you do not feel able to discuss it with us in person, you can contact our Data Protection Officer, Mr Muller van der Westhuizen via email pddlaboratory@gmail.com or in writing by letter. We take complaints very seriously.

If you are unhappy with our response or if you need any advice you should contact the Information Commissioner’s Office (ICO). Their telephone number is 0303 123 1113, you can also chat online with an advisor. The ICO can investigate your claim and take action against anyone who’s misused personal data. You can also visit their website for information on how to make a data protection complaint.

 

Disclosure

We may need to disclose your information. In order to provide proper and safe dental care to:

  • Other health professionals caring for you including but not limited to your general medical practitioner and the hospital or community dental services.

  • The Inland Revenue.

Disclosure will take place when relevant on a need to know basis. This means only those individuals or organisations that need to know in order to provide care to you and for the proper administration of Government will be given this information. Only information that the recipient needs to know will be disclosed.

In very limited circumstances or when required by law of a court order, personal data may have to be disclosed to a third party not connected with your health care. In all other situations, disclosure that is not covered by this Code of Practice will only occur when we have your specific consent. Where possible you will be informed of these requests for disclosure.
